Complete a Salary Reduction Agreement
To complete your new plan enrollment, please download the Salary Reduction Agreement by visiting your institution's Web site listed above. Or, you can pick up and complete a form at your HR/Benefits office.

How to transfer funds to TIAA-CREF
If you currently have assets with another plan provider and would like to transfer those funds to a TIAA-CREF plan, you can do so by completing a TIAA-CREF Transfer/Rollover form. A Transfer/Verification form is also required, which you can obtain from your HR/Benefits office.

There may be tax consequences associated with the transfer of assets. Indirect transfers may be subject to taxation, surrender charges, and penalties. Consult with your own tax advisor regarding your particular situation.
